enamel-commons names historia clinica as the INITIAL build focus and it does not exist in the app at all. DB side is partly there and empty: pacientes (0 rows, identity pair documentoType + normalized documentoNumber enforced by ux_pacientes_documento_live), antecedentesCatalog (0), pacienteAntecedentes (0). No route, no server action, no form reads or writes any of them.
