Historia clinica: mental-health RBAC tier + daily-recording requirement (Ley 26.657)

Ley 26.657 (mental health) requires daily recording for mental-health entries and an extended/reinforced-confidentiality authorized-accessor list distinct from general HC access. Enamel's current RBAC (groups/userGroups) has no per-record-sensitivity tier - it is app/appKey scoped, not record-type scoped. Needs a decision on whether antecedentesCatalog's existing grupo column becomes the sensitivity discriminator or a new mechanism is needed, before any mental-health-flavored antecedente is offered through the v1 picker.
