Historia clinica: 10-year retention floor has no enforcement mechanism

Ley 26.529 sets a retention floor of >=10 years from the LAST entry, never hard-delete before that. pacienteAntecedentes already uses soft-delete (deletedAt) for the 'erroneous' close path, but nothing enforces the 10-year floor - a soft-deleted row can be hard-purged today by any future cleanup job with no gate checking it. Needs a constraint or documented invariant before any purge/retention-cleanup tooling is built against this table.
