EVO-57: audit-trail schema plan (coverage-scoping + field-name normalization)
Plan-only per Elazar directive (nqla #5+#6), relayed pmmaster 2026-07-17. Deliverable = plan doc (bwi ref + approach + migration sequencing), NO DDL. Two dimensions: (A) COVERAGE - createdBy/At + deletedBy/At + changelog/archive added to SENSITIVE-data tables only; EXEMPT telemetry (appEvents/friction views), system-queue, and non-sensitive lookup/catalog tables (Elazar #5a). (B) NAMING - normalize audit/soft-delete/changelog/historial column-naming to one convention fleet-wide; integrate existing historial mechanism, do not duplicate. Input = VENUS-265 (EVO-55 gap report): only lookupOptions+practicas have createdBy; zero tables have deletedBy; only practicas+practicaParticipations have archive-trigger coverage; practicas trigger misses INSERT. PM drafts WITH coder-venus-cc + db-venus-cc + audit-venus-ca.

(B) coder-venus-cc: no historial table exists (UI label only), extend archive-trigger primitive not duplicate; standardize WHO axis on <pastVerb>By uuid FK, add deletedBy fleet-wide, retire ...ByUserId, keep BySessionUser as distinct DB-role axis. audit-venus-ca PASS with scope-lock caution on Stage 2 (practicas INSERT-trigger fix must not carry a rename pass). Relayed to pmmaster.

EVO-57 HOLD + re-scope (Elazar steer via pmmaster, 2026-07-17): no blanket audit-trail coverage. Every added archive twin/createdBy/deletedBy/updatedBy column now needs per-table justification, not coverage-for-coverage. Tempers EVO-55's original 'every table' ambition. DEFERRED, no release GO. Plan doc (shared/reports/2026-07-17-venus268-audit-trail-schema-plan.md) stays as reference but Stage 1-4 as drafted will need re-scoping to minimal-justified-set when Elazar picks it back up.
