Complete sede visibility and delegated evaluator attribution in practice exports

Client request: make sede visibility selectable in the evaluation/practice workspace as well as in exported PDFs; add the delegated evaluator attribution to the single-practice ficha PDF. Existing code already supports excluding sede from PDF and stores the actual evaluator plus delegated display name in append-only evaluation records; those remain intact. Scope only the missing presentation paths. Roster name provenance is the tracked client-provided document dated 2026-10-07; no email attachment was available here for comparison.
