Rename práctica date label to "Fecha de la cirugía" + show load date "Cargada el"

Rename práctica surgery-date label "Fecha" → "Fecha de la cirugía" on every surface (create/edit form, detail view, both PDF variants, correction UI) + additionally display the load date separately as "Cargada el {fecha}" (from práctica.createdAt) in detail view + PDF so the two dates are unambiguous. Class-S. No schema change (createdAt exists), no data migration. Root evidence: ~30 prácticas used the comment field to record the real surgery date because "Fecha" was ambiguous; client uses "cirugía" for the procedure so the label matches his wording. Was mislabeled VENUS-295 in dispatch (that id went to G8/G10); this is the real R3 id.
