feat(monitoring): venus EVO-12 leg — server-side 404 capture, own-route 404 -> ERROR tier, bot-probe -> WARN/digest carve-out; emits via the dormant flag-gated 404 email path (NOT instrumentation.ts, which notFound() does not throw to). Per EVO-12 canonical spec (pm-llmmsgsrv coord).
