crearPracticaFailed INSERT column/value mismatch

practica-actions.ts INSERT had 23 named columns but 22 value-expressions (imagenes column had no placeholder). Copy-paste drift bound now() to complicacionesIntraNotas instead of complicacionesCompletedAt, cascading every later placeholder down one and dropping the last. Also fixed: pacienteDob cast ::uuid should be ::date, pacienteId cast ::jsonb should be ::uuid, pacienteSexo cast ::jsonb should be plain text. Class-S app-code fix. SHA f229f79/v0.95.25, live-version verified, tsc clean, corrected INSERT tested in rolled-back txn against prod DB. Correlates with SOP-15 (Daiana Rodriguez, opened 7min after the logged error, same repro: cannot cargar prácticas).
