terra_38: módulo-1 campos over-offer optional fields on 3/10 cirugías

terra_38 (TERRA-96) defines terraModuloCampos per-MÓDULO not per-cirugía. Módulo 1 has 10 cirugías; injerto+barrera fields apply to only 7 of them (the bloque ones) while Cigomáticos and Pterigoideos carry marca only. Because campos are scoped to the módulo, terra's UI offers tipoInjerto/tipoBarreraImplante as optional fields on all 10 módulo-1 cirugías, including the 3 where the client spec never asks for them. Fields are isRequired=false so no data-integrity violation, but the form over-offers input the client spec does not define for those 3 rows. Documented in terra_38 header at authoring time. No fix shipped - filing per coder-terra-cc instruction not to hide this gap.
