terra_33: provisional seed of the four modulo-campo vocabularies (tipo de injerto, zona dadora, tipo de barrera, marca de implante), marked for catedra review

ELAZAR RULING 2026-08-22 (CLI, "3b"): seed a PROVISIONAL list for each of the four categories from the literature, marked for Puia review - the client has not answered and the four fields have been live-but-disabled since 0.24.14. This overrides the TERRA-76 "lists are the catedra to supply" hold for the SEED only; the no-free-text rule stands. PROVENANCE MARKING: lookupOptions has no isConfirmedByClient column (shared table, terra cannot add one), so provisional status is carried in (a) this migration header, (b) the app - PROVISIONAL_CAMPO_CATEGORIES in src/lib/lookups.ts renders "lista provisoria, a confirmar por la catedra" under the picker, removed per category when Puia confirms - and (c) pm/05-client-info.md OPEN items. All four rows: ownerAppKey=terra, appKey-scoped via the existing include-mode vocabularyScopes row from terra_31, one vocabularyScopeMembers row per value. Labels es-AR, values camelCase.
