JTP bell notification lost on práctica-create pool timeout

practicaNotificationFailed (system, error) 2026-08-11T23:45:24Z: pool-acquire timeout (pool.ts max=2, connectionTimeoutMillis=5000) during createNotification post-commit fan-out on práctica create. Not a PLUTO-688 instance — in-app bell table only, no email/outbox involved. Fixed: reuse the already-held transaction client instead of re-acquiring for the display-name lookup; a failed name lookup now degrades to a generic label instead of skipping the notification outright. Shipped 3d262e9 / v2.22.80. Coder mislabeled this PLUTO-692 in FINISHED reporting before filing it — retro-filed here, correct ID recorded.
