lookupOptions: add updatedAt + touch-trigger (sweep-found hygiene)

Sweep-found hygiene (bs-mrtkq6rjomf dual-axis information_schema sweep, ratified fleet hygiene). SEPARATE from PLUTO-625, rides alongside, not blocking. lookupOptions is admin-edited IN PLACE (activo toggle + label/params edits) but has createdAt and NO updatedAt. Add updatedAt timestamptz default now() + touch-trigger. VERIFY FIRST before finalizing exemptions: the 6 practica-detail child tables (practicaAnestesias/AntecedentesPersonales/CaracteristicasPieza/ComplicacionesIntraoperatorias/DiagnosticosRadiograficos/TecnicasQuirurgicas) are believed replace-on-edit (delete+reinsert on rectificar -> insert-only, createdAt correct); confirm the rectificar server-action path does NOT UPDATE them in place. If any IS updated in place, it joins lookupOptions in this WI. Class-A additive DDL, db-pluto lane.
