Report top sources to PM for root-cause+fix dispatch (coder, Class-S unless auth/schema/email). Reference: prior Mars campaign cut toasts hundreds/wk -> <12 by fixing UI, not suppressing logs.

SHA 3dc95f5 v2.20.13 (Class-S, coder self-verified live). Fix scope after recency correction (185/230 initial friction was a pre-541 fix-date tail, not live): partidoLocalidad required->optional (nullable column, admin not clinical) — was the only genuinely-live intake offender, expect ->0. Escape-first reorder + inline 'Sin X' hint on antecedentes/caracteristicas/complicaciones (decoration only, no auto-default per data-quality boundary). sexoPaciente/dificultadPercibida/DOB-invalida native-required events confirmed dead tail from PLUTO-541 noValidate change, no fix needed, decoration skipped per pmmaster override. Server-side Error al crear/actualizar (10 events) already fixed by a4e4fd5/PLUTO-573 on 07-17, not live.
