Phase 4 (db-pluto-cc DDL + coder call-sites): fix the two naming outliers to the canonical convention — studentDeletedAt (prefixed deleted-at) and the bare 'reason' columns (2 tables) → named per convention. Rename migration + update every read/write call-site. Low row-volume, sequence after phases 1-3 to avoid churn overlap. EVO-57 audit-trail schema program; plan authored by coder-pluto-cc from PLUTO-580 gap report (db-pluto-cc). Elazar-gated before DDL.
