Additive migration adding missing lifecycle columns on sensitive base tables, starting with the 8 practica* clinical-detail tables (all lack createdBy) and accessRequests/supportTickets/supportTicketMessages/users/lookupOptions/systemSettings (1-2 missing fields each). Historical unknown actors stay NULL, never fabricated. New-write enforcement deferred until all writers supply actors. Class-A DDL: design-ping+audit+full-team agreement before push.
