blackboard: decline as a third outcome, decliner stays withheld forever

Today a respondent has two states, written or silent, so a considered "I have nothing useful here" is indistinguishable from a dead session - which is why coder-mars-cc spent a blind nudge on an agent that had already answered. Op: blackboard_decline(entry_id, reason optional, max 300 chars). HARD CONSTRAINT from coder02-mars-cc, and it is the reason this is safe: A DECLINE MUST NOT UNLOCK READING. The decliner counts toward completion and stays PERMANENTLY WITHHELD on that entry - mayReadAll must not become true for it at close either. Without that clause decline is a one-word bypass of the write-before-read gate and every agent under time pressure will find it. The decline is visible to the opener in the roll call (a third bucket beside written and pending) and named in the completion and timeout notices, so the reason is recorded but never counted as an answer.
