Sweep for other per-request/standing-condition alerts that should use logEventOnce

coder-mars-cc shipped logEventOnce(input, window) in src/lib/logger.ts for MARS-617 (atomic insert-if-absent appEvent for a STANDING CONDITION, e.g. dedup a warn that would otherwise fire every request while the condition stays true). Coder flagged, unswept: are there other alert sites logging per-request off a condition that stays true rather than a discrete event? Sweep and convert candidates to logEventOnce.
