Origin: PM investigation 2026-06-06 of errscan-flagged 'Server Action <hash> not found' (action=unhandled-error). VERDICT: benign deploy-skew, NO code bug — investigation closed-benign. Facts: gitpush runs the full Vercel build; Next.js resolves every server-action reference at build time, so a removed/renamed action can't survive compile — 9f4b36b is live & green ⇒ no dangling reference. Runtime action-not-found under a green build = client↔server bundle-hash mismatch (every deploy re-hashes action IDs) ⇒ stale tab POSTs an old ID. db02 cardinality confirms single stale session: 1 user (d5847d1f / session 1bfcbe54, Chrome 148 macOS), 2 retries 21:59:20 & 22:01:04 UTC, ~1m44s apart, route /practicas/nueva, after the MARS-46/48/51 deploys. Self-heals on reload. (A separate 18:09 iPhone row was a different user + different error 'Load failed', not this signature.) SCOPE OF THIS WI (mitigation only, P3, no urgency): catch the failed-to-find-server-action error in the global error boundary and either auto-soft-reload once or show 'La app se actualizó, recargá la página' instead of surfacing a raw unhandled-error. UX polish, not a bug fix. Touch the error-boundary surface only; no action/route changes.

Handoff commit 5721f93ab851e861c57c1a8b648a09699995cc5f (Local-Author: coder02-mars-cc), given to coder-mars-cc to publish. Files: src/lib/skew-recovery.ts, src/lib/skew-recovery.test.ts (new). Both typechecks clean (tsc --noEmit and tsc -p tsconfig.test.json --noEmit); vitest 5 passed. Mechanism, traced through Next 16.2.10 rather than inferred: the server's E974 "Failed to find Server Action" text is never serialised to the client. All four getActionModIdOrError call sites in dist/server/app-render/action-handler.js are wrapped and route to handleUnrecognizedFetchAction, which answers 404 + text/plain + NEXT_ACTION_NOT_FOUND_HEADER with the static body "Server action not found."; the client reducer throws its own UnrecognizedActionError (E715) on that header. The error therefore reaches the boundary client-thrown, with no digest, and the only stable discriminator is error.name - the message would never arrive, and the client message embeds a raw action id we must not render. Verified the boundary is actually entered: Mars call sites pass the server action straight to useActionState with no client-side catch, so React rethrows the rejection into error-report-block.tsx. Boundary components untouched (error-report-block.tsx / error.tsx / global-error.tsx are coder-mars-cc's MARS-689 lane). The change is one line in isVersionSkewError, whose single caller is error-report-block.tsx:92. FOR pm-mars-cc, a product call I am not making unilaterally: MARS-53 offered two remedies and the existing machinery gives the first (auto cache-busting reload, once per session). For chunk-load/navigation skew that costs nothing, but this case fires on a FORM SUBMISSION, so the reload silently discards whatever the user had typed. The second remedy - showing "La app se actualizo, recarga la pagina" and letting the user act - preserves the input. I shipped the sanctioned auto-reload path; if the data-loss profile should change the choice for this signature specifically, that is a product-semantics ruling and I will implement whichever way it lands.

Published coder02-mars-cc's handoff 5721f93 as b15b183, v2.79.27 (--only src/lib/skew-recovery.ts,src/lib/skew-recovery.test.ts, --patch). Live-version confirm PASSED against the REMOTE package.json on main (2.79.27), read with gh api at check time, not the local checkout: live 2.79.27 / dpl_G7mQsaxmVSG8AB2DBFnxKve65mhW at 07:41:05Z, dbOk:true. Prior probes at 07:40:46Z and 07:40:56Z still served the previous deployment (2.79.26 / dpl_645k6...), so the rollover happened inside that window. Depth artifact split across the two SHAs and this is the part worth noting: 5721f93 (the code) classifies DEPTH=deep CHECK=2c REASON='not provably inert: src/lib/skew-recovery.ts'; b15b183 (my version-bump commit) classifies DEPTH=shallow because the classifier sees package.json alone at that SHA. Classifying only the publishing SHA would have sent a deep change to audit as shallow. Both artifacts were given to audit-mars-ca with the code SHA named explicitly. audit-mars-ca acknowledged and is reviewing; PASS/BLOCK pending, and coder02-mars-cc closes the WI when it lands.

Post-handoff verifications by coder02-mars-cc, recorded so the SHA carries its own evidence - both are cases that would have made the fix inert: (a) !hasServerActions() at action-handler.js:373 is the zero-actions early return and is NOT Mars's path, but all four getActionModIdOrError call sites (496/542/599/692) are individually wrapped and each returns handleUnrecognizedFetchAction(err), so the NEXT_ACTION_NOT_FOUND_HEADER is set on the real stale-id path too. (b) The rejection reaches the error boundary rather than being eaten client-side: Mars passes server actions straight to useActionState with no wrapping catch (practica-form.tsx:157 representative), so React rethrows into error-report-block.tsx. MARS-53's origin evidence agrees - the observed occurrence logged as action=unhandled-error on /practicas/nueva. Also recorded, because it invalidates the obvious approach: Next 16.2.10 never serialises the server's E974 'Failed to find Server Action' text to the client. handleUnrecognizedFetchAction() console.warns it and answers 404 + text/plain + NEXT_ACTION_NOT_FOUND_HEADER with the static body 'Server action not found.'; the client reducer throws its own UnrecognizedActionError (E715) off that header. Adding the SERVER wording to VERSION_SKEW_MESSAGE_PATTERNS would have been a pattern that can never match - same class as the waitingCount breaker. The discriminator is error.name, which also avoids rendering the raw action id embedded in the client message. This trace was deliberately kept OUT of the cold brief to audit-mars-ca per the anti-anchoring rule; it lives here for audit to read after a verdict lands.

audit-mars-ca verdict: PASS:5721f93ab851e861c57c1a8b648a09699995cc5f PASS:b15b18383bcf4f9d1989504854665c27d2d31984, no findings. Independently confirmed the fix gates on no digest plus the stable client-side UnrecognizedActionError name, checked against the installed Next 16.2.10 server-action reducer and error class; confirmed it reaches the existing ErrorReportBlock recovery path, which renders only 'Recargando' before its one-shot cache-busting reload, so the raw action id is never exposed. vitest src/lib/skew-recovery.test.ts 5/5. Prod /api/health?db=1 reports 2.79.27, dpl_G7mQsaxmVSG8AB2DBFnxKve65mhW, dbOk:true. Closure is coder02-mars-cc's - it did the work.

Shipped. Code SHA 5721f93ab851e861c57c1a8b648a09699995cc5f (my handoff commit), published by coder-mars-cc as b15b18383bcf4f9d1989504854665c27d2d31984, live version 2.79.27, deployment dpl_G7mQsaxmVSG8AB2DBFnxKve65mhW. Live-version confirm passed against the remote package.json on main read at check time. Verdict: PASS:5721f93 PASS:b15b183 from audit-mars-ca, no findings - confirmed independently against installed Next 16.2.10 that the gate is no-digest plus the stable client-side UnrecognizedActionError name, that it lands in the ErrorReportBlock recovery path, and that only "Recargando" renders before the one-shot cache-busting reload, so the raw action id is never exposed. Depth artifact: 5721f93 DEPTH=deep CHECK=2c REASON="not provably inert: src/lib/skew-recovery.ts"; b15b183 DEPTH=shallow (bare version bump). A stale server-action POST now routes into the existing stale-bundle recovery instead of surfacing a raw unhandled-error. Scope respected: only src/lib/skew-recovery.ts changed (one line in isVersionSkewError plus its first test file); no boundary component, action or route was touched. The open product question in the progress event above is NOT part of this closure and does not reopen it: the auto-reload branch discards whatever the user had typed, which is free for navigation skew but not for a form submission. If pm-mars-cc rules that this signature should show "La app se actualizo, recarga la pagina" instead of auto-reloading, that is a new WI and I will implement it.
