The 3 noRevisado-with-aprobado-review prácticas are valid aprobado->teacher-overrode->rectificar->student-resubmitted sequences (all María Bautista Muñoz), currently noRevisado awaiting her re-review. The aprobado row is legit overridden history; practicas.approvalStateOptionId correctly noRevisado. Detection query was too broad (fired on any aprobado review row); correct integrity check = noRevisado AND LATEST review = aprobado (zero occurrences in prod today). No code fix, no row repair — user-workflow item, not an app bug.
