  Triggering case: SOP-16 / Tobias uploaded wrong radiographs on a práctica that was already graded. STUDENT_EDITABLE_PRACTICA_APPROVAL_STATES=[noRevisado,rectificar] (src/lib/practica-status.ts), so a graded práctica is locked to the student AND there is no JTP/operator action today to move graded→rectificar. Tobias is being unblocked via a one-off audited DB state revert (db-mars). Durable fix: JTP-facing 'solicitar rectificación / reabrir' server action that flips approvalState graded→rectificar through the app, audited via setArchiveContext (practicas is an audited table), reusing PRACTICA_APPROVAL_STATES.
