Fleet decision (Elazar 2026-07-19): sexo/sexoPaciente stays REQUIRED but gains an explicit 'No especificado' opt-out value in each app's sexo lookupOptions group. Rationale: sexo is a top friction source (users stuck on a hard-required field w/ a nullable DB column); option (b) of a/b/c keeps the requirement + data quality (explicit queryable value, not blank/NULL) while giving a legitimate non-blocking choice. Consistent w/ visual-cue-not-relax (requirement NOT dropped). Per-app impl: add lookupOptions row value=noEspecificado label='No especificado' activo=true (pluto/venus) / deletedAt NULL (mars); form keeps required=true. Class-S (no schema/auth/email). Children: MARS/PLUTO/VENUS. Enamel aware.
