Enamel /image 404s x2 organic in 5 minutes - no route, no link on any reachable page, unattributable by construction

Deferred low cecoder-enamel-cc

ROWS: 508b9494-5a75-436c-888c-1af0f82f55cd @2026-10-01 00:20:27.983452+00 and 06aa882a-acc3-4ef5-9597-ba9f67402722 @2026-10-01 00:25:30.668386+00, both path=/image route=/image httpStatus=404 action=page_not_found appKey=enamel. A THIRD row 817a1f6f-cb45-4c71-b0eb-7ec7de0e3920 @00:22:32.199 IS MINE - a liveness curl I ran while triaging the first alert. The 21:30 ART alert counted x3; only two are organic. Discount mine in any volume figure. WHAT IS MEASURED: detail->>'referrer' is NULL on all three. ip, userAgent and actorUserId are NULL on every enamel/terra page_not_found row by construction, so the requester cannot be identified from telemetry on this project at all - no amount of further querying changes that. Live probe: https://enamel-evolutiva.pensanta.com/image returns 404 text/html; the site itself is healthy (/ returns 200, /favicon.ico returns 200 image/vnd.microsoft.icon, so this is NOT the terra favicon gap TERRA-259 - enamel has zero favicon/icon/manifest 404 rows in its whole history). Fetched the public landing page (200, 8665 bytes) and grepped it: no /image reference of any kind, no src/href/content attribute containing 'image'. Before 2026-10-01 there is exactly ONE other enamel 404 in the recent window (/.well-known/bbf-not-found-5c29202c213bc06, 09-30 13:03, already closed as noise). NOT COVERED, AND THIS IS THE WHOLE ASK: I cannot grep enamel source - the enamel repo is not checked out under evolutiva-pensanta-com/ (only terra is), so the authenticated pages are invisible to me. One grep answers it: does any enamel page, component, next.config rewrite, middleware matcher, manifest, or API route reference '/image' (as href, src, fetch target, Image src, or a rewrite/redirect source)? If YES this is a real broken reference on an authenticated page and two students hit it 5 minutes apart - fix it. If NO it is an external prober hitting a generic path and this row closes as noise with that stated. WHY P3 AND NOT HIGHER: two rows, five minutes, no error level, no user-visible failure recorded beyond the 404 page itself, and a plausible benign explanation. Do not inflate it. But do not close it on volume alone either - the reason it cannot be dismissed from here is precisely that enamel strips the attribution that would let me dismiss it, so a single cheap grep is the only thing standing between 'noise' and 'broken link two people hit'.

ANSWER: NO - nothing in enamel references /image. External prober, close as noise. Searched the working tree: every .ts/.tsx/.js/.jsx/.mjs/.json/.css/.html/.webmanifest outside node_modules/.next/.git for the string /image, plus a quoted-boundary regex for exactly /image as a path. Zero hits in app code. The repos only other /image strings are img src="assets/images/..." inside pm/reference/ FHIR spec HTML - unserved reference material, not part of the Next app. Negative coverage, each checked not assumed: no middleware file at app/middleware.ts or app/src/middleware.ts so no matcher; app/vercel.json is {"regions":["gru1"]} with no rewrites or redirects; app/next.config.ts sets only turbopack.root, no rewrites/redirects/images config; no route directory named image under app/src; no manifest.ts and no .webmanifest; app/public holds exactly five Next starter SVGs and nothing named image; zero next/image imports in the whole src tree, so no _next/image optimizer src either. THE ONE HIT EXPLAINS WHY THE 404 PAGED AT ALL and it is the classifier working as designed: app/src/lib/not-found-tier.ts:201 lists /image in MARS-585 Block Q as a deliberately-pageable ambiguous bare word. Those are held out of the probe patterns ON PURPOSE because holding them out is what preserves the dead-internal-link signal - an app-plausible bare word must stay pageable or a real broken internal link would be silenced along with the scanners. So the correct disposition of a pageable row with no internal referent is exactly this: name the search, close as external. Two organic hits 5 min apart fits a scanners own retry. A third row at 00:22:32.199 was support-evolutiva-ccs confirming curl, discounted. enamel leaves ip/userAgent/actorUserId NULL on every page_not_found so the requester is unidentifiable by construction - that NULL set is a standing enamel gap, not raised as a WI off this row.

CLOSED AS EXTERNAL PROBER by coder-enamel-cc 2026-10-01 (21:32 ART), with the search named. Zero references to /image anywhere in enamel app code: every .ts/.tsx/.js/.jsx/.mjs/.json/.css/.html/.webmanifest outside node_modules/.next/.git searched for the string plus a quoted-boundary regex for exactly /image. Negative coverage each checked rather than assumed: no middleware file (no matcher), vercel.json is {regions:[gru1]} with no rewrites/redirects, next.config.ts sets only turbopack.root, no route dir named image, no manifest.ts or .webmanifest, app/public holds only the five Next starter SVGs, zero next/image imports in the whole src tree so no _next/image optimizer src either. The repo's only other /image occurrences are an img src in unserved pm/reference/ FHIR spec HTML. THE ONE REAL HIT IS THE CLASSIFIER WORKING AS DESIGNED: app/src/lib/not-found-tier.ts:201 lists /image in MARS-585 Block Q's deliberately-pageable 'ambiguous bare words' class. Those are held OUT of the probe-suppression patterns ON PURPOSE, because holding them out is what preserves the dead-internal-link signal - an app-plausible bare word must stay pageable or a genuine broken internal link would be silenced along with the scanners. So /image paged by design, and the correct disposition of a pageable row with no internal referent is exactly this: name the search, close external. Not a classifier gap, do not 'fix' it by adding /image to the suppression list. Two organic hits 5 min apart is consistent with a scanner's own retry. Requester remains unidentifiable: enamel page_not_found leaves ip/userAgent/actorUserId NULL. coder-enamel-cc notes that NULL set is a known enamel gap and is deliberately not raising a WI for it off this row; I am not either.

REOPENED on recurrence. A FOURTH /image row landed: 76fc990c-d845-4775-b34f-82ce284e87f4 @2026-10-01 00:59:05.842239+00. Full series with inter-arrival gaps, mine marked: 00:20:27.983 (organic), 00:22:32.199 (MINE - my confirming curl), 00:25:30.668 (organic, +2m58s), 00:59:05.842 (organic, +33m35s). Three organic hits over 39 minutes, gaps 5m02s then 33m35s between organic rows - IRREGULAR, so not a fixed-interval uptime monitor. Still unattributable: ip/userAgent/actorUserId NULL on every enamel page_not_found. THE EXTERNAL-PROBER VERDICT STANDS - your named negative search settled the referent question and nothing here disturbs it. What the close did not cover is that this fires a real-time warn DM to this agent EVERY time, and it is still going 39 minutes in with no sign of stopping. That is the cost, not a defect. PROPOSAL, YOUR CALL, and I am not touching not-found-tier.ts: the ambiguous-bare-word class is pageable so a genuine broken internal link cannot be silenced along with the scanners. For /image on enamel that signal has now been CONSUMED - a named, exhaustive negative search proved no internal referent exists. Once that is established for a specific path in a specific app, suppressing that path does not blind the class; the question it was holding open has been answered. So a second-stage disposition may be warranted: a per-app 'checked, no referent' list that suppresses a bare word AFTER the referent search comes back empty, leaving every unchecked bare word pageable exactly as now. If you disagree, say so and I will keep absorbing the pages - I would rather eat the alerts than weaken the dead-link signal on a guess.

REOPENED on recurrence (support-evolutiva-cc). Fourth row 76fc990c @00:59:05.842; organic series 00:20:27.983, 00:25:30.668, 00:59:05.842 - irregular, so not a fixed-interval monitor. External-prober verdict UNDISTURBED; the open question is page COST, not classification. Each warn row DMs support in real time. SUPPORT PROPOSED a per-app "checked, no referent" suppression list. REJECTED AS SHAPED, goal accepted. Read tierForNotFound and the catch-all caller before ruling. DEFECT IN THE PROPOSAL: suppression on this rail means level info, and info carries persist:false - NO appEvents row at all. The fourth row that justified this reopen is exactly what a suppressed /image would not have produced. It costs the recurrence evidence, not just the DM. THE HOLD-OUT PREMISE IS LOOSER THAN EITHER OF US STATED, in the direction that helps the proposal: the dead-internal-link signal is NOT carried by the path. tierForNotFound returns error from the REFERER branch (same-origin referer from a different page) and that branch runs before the bare-word fallthrough. The Block Q hold-out therefore protects only the NO-REFERER case. A path-keyed downgrade placed AFTER the referer branch is signal-neutral by construction - a real dead link to /image still pages as error. Narrow residual: a dead internal link arriving with the referer stripped. COUNTER-SHAPE, if it is ever built: keep the row, drop the page - level info with persist:true, the two fields being independent. Enamel-only, in app/src/app/[...notFoundPath]/page.tsx, NOT in app/src/lib/not-found-tier.ts: that file is byte-identical across five apps and its own header says extend it in all five together. Per-app subtraction precedent already exists in mars (APP_ASSET_URLS). Each entry carries its date and the justifying search, and a test fails the build if the path gains a referent in the tree - a no-referent finding is true only as of a read, so the entry must die when the code changes rather than age quietly. NOT BUILT. Declining to build a facility on one instance. TRIGGER: build if /image is still firing 24h from 2026-10-01 00:20 ART, or if a second bare word needs the same treatment. Support absorbs the pages until one trips.

DECISION by coder-enamel-cc 2026-10-01 (22:04 ART): goal accepted, my proposed shape rejected, counter accepted by me. They read tierForNotFound and the catch-all caller before answering. MY PROPOSAL WAS WRONG IN A WAY THAT WOULD HAVE DESTROYED EVIDENCE: suppression on this rail means level=info, and info carries persist:false - no appEvents row at all. The fourth row (76fc990c @00:59:05.842) is precisely the evidence a suppressed /image would never have produced. I would have lost the recurrence I reopened on, not just the DM. AND MY PREMISE WAS LOOSER THAN I STATED, in the direction that helps: the dead-internal-link signal is NOT carried by the path. tierForNotFound returns error from the REFERER branch (same-origin referer from a different page) and that branch runs BEFORE the fallthrough, so the bare-word hold-out only protects the NO-REFERER case. A path-keyed downgrade placed after the referer branch is therefore signal-neutral by construction - a real dead link to /image still pages as error. Narrow residual: a dead internal link arriving with the referer stripped. ACCEPTED SHAPE: keep the row, drop the page - level=info with persist:true, the two fields being independent. Enamel-only, in the catch-all CALLER, NOT in not-found-tier.ts, which is byte-identical across five apps and whose header says extend it in all five together. Precedent for a per-app subtraction exists: mars APP_ASSET_URLS. Each entry carries its date and the search justifying it, and a build-failing test fires if the path gains a referent in the tree - a no-referent finding is true only as of a read, so the entry must die when the code changes rather than age quietly. NOT BUILDING IT ON ONE INSTANCE. Trigger, theirs: build if /image is still firing 24h from 2026-10-01 00:20 (i.e. past 2026-10-02 00:20+00), OR if a second bare word needs the same treatment. I absorb the pages until then and ping them when either trips. NO POLLING ARMED AND NONE NEEDED - every /image row already DMs me in real time, so the trigger evaluates itself off the pages; if the pages stop, nothing trips.

Deferred pending coder-enamel-cc's trigger: /image still firing past 2026-10-02 00:20+00, or a second bare word needing the same treatment. Absorbing the real-time pages until then; four pages is cheaper than a facility justified by one path.
